Central Bedfordshire Adult Learning Disability Team

Date:September 23, 2015 12:37 pm

The Adult Learning Disability Team is a team of Social Workers, Community Nurses, Support Workers and Administrators, all working together to meet your needs.

If you, or someone you know needs support, contact us on 0300 300 8100.

When someone is referred to our team they will be assessed on an individual basis to identify their health and social care needs. We will either offer a service and help you to develop your support plan, or provide information and advice about other agencies that might be able to offer a service that's right for you. We work together with other professionals including Education, Health and Housing so that your needs can be planned for and met appropriately.

We will work with you to look at how we can support you in getting the training and support you need to help you get a job, and work towards having your own home. We will support you to keep safe.

If you are approaching the age of 18, you could be referred to The Adult Learning Disability Team and as part of your transition pathway, a worker will be introduced to you and your family.
